What Your Ferritin Level Reveals About Iron and Inflammation

Ferritin reflects your iron stores — but also rises with inflammation. Here's how to read low and high levels, and why context matters so much.

Ferritin is your body's iron savings account — a low level usually means your iron stores are running down, while a high level can mean either too much iron or, just as often, inflammation. Reading it correctly is all about context.

Ferritin is the protein that stores iron. Because it's also an "acute-phase reactant" that rises with inflammation, the same number can mean very different things depending on the rest of your picture. This is educational content, not medical advice.

The short version

  • Ferritin reflects iron stores — but also rises with inflammation, infection, and liver issues.

  • Low ferritin (often below ~30 ng/mL) strongly suggests iron deficiency, even before anemia appears.

  • High ferritin can mean iron overload or simply inflammation — context decides.

  • It's best interpreted with other iron studies and an inflammation marker.

Reading low ferritin

A ferritin below about 30 ng/mL is a strong indicator of iron deficiency — and symptoms like fatigue, hair thinning, poor exercise tolerance, or restless legs can appear before anemia shows up on a standard CBC. In menstruating women, endurance athletes, and people with gut issues, low ferritin is common and very treatable.

Reading high ferritin

Because ferritin climbs with inflammation, a high value isn't automatically "too much iron." Persistently high levels (well above the lab's range) can point to genuine iron overload (such as hemochromatosis) and warrant evaluation, but often a high ferritin simply reflects inflammation — which is why pairing it with hs-CRP and other iron studies matters.

What to do

The right response depends entirely on the cause. Low ferritin may call for dietary iron or supplementation and a look for why iron is low. High ferritin may need further iron studies or evaluation for overload or inflammation. This is very much a clinician's call — iron supplementation without a confirmed need can be harmful.

Which provider and what to ask

Primary care and hematology handle this. Ask: Is my ferritin low enough to suggest iron deficiency? If it's high, could inflammation explain it, or should we check for overload? What other iron studies do I need?

Frequently asked questions

What is a normal ferritin level?
Ranges vary by lab and sex, roughly 12–300 ng/mL for men and 12–150 for women; below ~30 suggests deficiency.

Can I have low iron with a normal CBC?
Yes — ferritin often falls before anemia appears, so you can be iron-deficient with normal hemoglobin.

Should I just take iron if mine is low?
Only with a clinician's guidance — the cause matters, and too much iron is harmful.

Key takeaways

  • Ferritin reflects iron stores but also rises with inflammation.

  • Below ~30 ng/mL strongly suggests iron deficiency, even without anemia.

  • High ferritin needs context — overload versus inflammation.
